Our coaches are here to foster a supportive and enjoyable environment. If a player is struggling, they might spend some time off-field receiving encouragement and support to regain their confidence.

Liability Waiver
Risk Acknowledgment
Soccer involves inherent risks. By signing this waiver, parents acknowledge these risks as part of their child’s participation.

Emergency Authorization
You authorize PLAY Soccer to manage emergency medical situations if needed.
Insurance Confirmation
You confirm that your child has health insurance, which will cover medical treatments if necessary.
Acceptance of Terms
Your signature on this waiver confirms that you understand and agree to its terms, which remain effective as long as your child is involved with our programs.

Consent to Use Images
By signing this form, you allow PLAY Soccer to use images of your child for promotional purposes, capturing the authentic atmosphere of our academy.
Withdrawal of Consent
You can withdraw your consent at any time by informing us.
refund policy
When Allowed
Refunds are not given on any classes already performed. A customer may request a refund for unused credits, and these will be granted on a case by case basis.
Administrative Fee for Refunds
All refunds are subject to a 10% administrative fee to cover the costs associated with maintaining our academy’s operations.
reporting absences
Report Absences by Wednesday:
To report an absence, please fill out the absence form available under “Our Community” on our website by Wednesday. If not reported in time, the session will be considered forfeited.

Transfer of Number
